I work for a DDS/MD. We have a CRNA that comes in on occasion and does our anesthesia services. We have a contract with this individual to pay her a flat fee for each case. Am I able to bill the insurance companies for her services since we have contract with her? does having a contract with her make her an employee? Can I bill 99155-99157 with the QZ modifier and her NPI # and her as the rendering for these lines?
