I have a patient that was seen in office by a breast surgeon (E&M) charge. It was determined pt needed a biopsy. Same day did an excisional breast biopsy (19120) in surgery center across the hall. The surgeon is an hospital employee and the surgical center is owned by the same hospital. Modifier 25 on E&M or Modifier 59 on biopsy. It would not be Modifier 57 as it was done the same, not one day prior. Any thoughts. Thanks
