Compliance in coding Infusions

CPT guidelines for infusions are time based. Started at xx, completed at xxx. How do you all handle infusions that do not follow the order? Example:
Medication ordered to run at 200 ml/hr over 5 hours (1000ml bag). Nurse documents start time at noon but marks it "stopped" at 1600 with TVI of 1000ml.
Example #2: Medication order states "infuse over 1 hour"; nurse start time is noon and stop time is given at 1245. Do you code these regardless of whether the order was followed, or do you downgrade/write off for compliance reasons?

I have reached out to our MAC for their input but wanted to see what you all do from a charging/coding perspective.
