10040 vs 114xx

Hi, one of my docs (internal medicine), performed removal of cyst on a patient and insurance denied so the patient has been calling to see if we can change the cpt. Here is the very brief procedure note:

"Under sterile conditions and 2% xylocaine, I incised the cyst sharply. The sample was sent to pathology. I closed the skin with 3 sutures of 4-0 nylon. Neosporin was applied and some nonstick dressing was used."

Path report states epidermal inclusion cyst.

I used 10040 because in the cpt book, if you go to removal, cyst, it lists 10040. Or should this be an excision code 114xx ? Now he is saying it should have said excision not incision. The date of this goes all the way back to Jan of 2017.

Affinity Denying CPT Code 10040 – Acne Surgery

I have some claims that Affinity Medicaid has denied for the 10040 (Acne surgery) because the dx code L70.0 (Acne Vulgaris), is saying is not correct or incomplete. Has anybody else received this denial? L70.0 has always been paid with the acne surgery. There is no clinical policy on their website. I asked the rep and they told me to speak to the biller, however I am the biller. There are about 3-4 claims outstanding since Feb 2018. I’m thinking to appeal it with medical records.

10040 or Excision series?

I have a provider who coded this as 10040. I’m thinking this is an Excision. Patient will return to have sutures removed. Input please? Thank you much!
Below I listed an excerpt of the procedure performed:

Sebaceous cyst

PROCEDURE:
" … a straight incision was made on the surface skin and cyst dissected off in total. The cyst was punctured and drained cheesy material during the dissection. No adhesion noted between subcutaneous tissue and cyst wall. Pathology was deferred as there was no clinical suspicion of atypia. The subcutaneous space was closed with a single figure of 8 knot using 3-0 vicryl. Skin was approximated with 2 simple interrupted sutures using 4-0 ethilon. The area was cleansed with normal saline and dressed with a bandage."
