CPT 43239 and CPT 45380- I need help please!

Hello Dear colleagues;
The following cpts are bill both together as both procedures need to be done on the same day and have the insurance pay for both procedures fully.

Right now some insurances are paying only 50% for the second one and some are not paying at all.

I need to find out is there a modifier that needs to be used, is there another type of prior auth that needs to be obtained. What needs to be done to get paid for both CPT codes if done on the same day because it NEEDS to be done on the same.

These 2 codes are freqneutly billed together and we need to figure out how to get them paid. Can anyone assist me with coming up with a solutions for this dilemma?

43239 with 43259

I need clarification. When these two procedures are done together and you bill them out with 43239, 43259-59 the only code that Medicare pays is 43259. Now I called Medicare and they indicate that another modifier needs to go on the 43239 and not a -59 or a -51. What is the proper way to bill for these two procedures? I’m leaning towards just not billing the EUS all together. any thoughts? Thank-you

43235, 43237, 43239 questions. Need Help!

I have some questions on some codes and I was hoping you could give me some education. For CPT 43235 does a specimen have to be collected to use this code or is this with/without?

For 43248 can a specimen be collected or is there a different code?

For 43239 is there a difference between biopsy and specimen?
