Westside Los Angeles Dentist Being Sent To Prison

A West Los Angeles dental practitioner was sentenced last week to forty months in federal jail for a health-care insurance fraud scheme during which he submitted phony billings for crowns and fillings that were not ever really provided to patients. Benjamin Rosenberg, 59, pleaded guilty in January to one federal count of health care fraud, in keeping with the U.S. Attorney.

reconstruction of external ear after being bitten off by dog

We have a 10 yr old boy who had his ear basically bitten off. He was seen by a doctor who placed a permanent suture and sutured his ear to the skin on his head to allow healing of the ear injury. He was told to come back in 6 months to a year to remove the suture and do a reconstruction. The original doctor retired and now my doctor is seeing him.

My doctor states he would need to relieve the suture and then modify/reconstruct the ear without using any flaps or grafts. He would most likely be able to close any gaps with just sutures.

I know i need to get authorization from the insurance, that is why we don’t want to use an unlisted code. I don’t think this qualifies as complex repair because it is not an open wound.

My question is this- what code do i need to use without using an unlisted code? Would it be 69300 otoplasty? Any help would be greatly appreciated.

Can you bill for UA done in office with only documentation being the results?

A patient came in to PCP’s office to do a urine sample- the MA did a urine dip. They billed for 81002 but I do not have documentation by a nurse or doc indicating the patient was here…. I know the patient was because the results are in chart. My question is are the urine results proper documentation to support billing 81002? At my previous practice the MA always did a note with why patient coming in for urine sample and what physician was in the office…etc. and included the results. Newer to auditing and needing some advice. Thank you!

Claims not being paid for Ultrasound (CPT 76700 and 93975)

I have been billing CPT code 76700 with CPT code 93975 for ultrasounds and using only the 59 modifier. Recently, Blue Cross Blue Shield and Humana have stopped paying for these. Is this because I don’t use modifier 26? Or perhaps we need separate diagnosis codes for each CPT code? Can someone please advise. Thank you.

CPT 91035 is not being paid by the majority of the insurance companies- Why?

CPT 91035 is not being paid by the majority of the insurance companies. We need to find out how to get this CPT code paid. Do we need to add a modifier, obtain additional prior auths? This is usually billed with CPT 43235. Even when its billed by itself it is being denied. Anyone knows what happen here?

Modifier for Dermatology Excisions and Repairs being billed together

As a general rule, and I correct in using -51 on the Excision code when both procedures are billed together in the same visit? Or should it be -59? I’ve been told either, but that doesn’t seem correct. Thanks for your help!! :)
