re-excision squamous cell carcinoma

Hi! Our patient had excision of a skin lesion by her primary care doctor. The path showed squamous cell carcinoma with 2 margins involved.

The patient was referred to us for re-excision. We did this, and the pathology came back negative with no evidence of tumor.

How should I code our portion? Do I use a benign code with history of SCC?

Coding Eccrine Carcinoma of Abdomen

Hello,

I’m having a hard time figuring out how to code a primary eccrine carcinoma of the abdomen. An eccrine carcinoma develops in the sweat glands and metastasizes fairly quickly to other areas of the abdomen. I came up with two codes for it:
C76.2 – Malignant neoplasm of abdomen or C49.4 – malignant neoplasm of soft tissue of abdomen. Anyone have any idea if one of these is right? Or if not, how to code it?? I appreciate any help you can offer!

I really need help. I code for a general surgeon. The procedure was an "excisional biopsy of right subareolar mass". The pathologist report states:
Invasive ductal carcinoma, .grade 2
Ductal carcinoma in situ, intermidiate grade.
I am lost on diagnosis code for this. It is also a male patient.
