CDE in pediatric endocrinology clinic

Hello,

I don’t know why, but I posted the below message in the forum and it was not visible anywhere except under my profile for my posts, so I’m trying again…

I work for a multi-specialty pediatric group and our endocrinology group has hired several Certified Diabetes Educators who are also dual certified as Registered Dietitians. I am looking to find some reputable resources for guidance in billing for their services. Not all of our pediatric patients have a diagnosis of diabetes yet, but are starting to show signs and often also are overweight so our CDE/RD are doing nutritional counseling and I guess you could call it preventive based education/counseling on the risks of developing diabetes.

Any help would be appreciated!

Infusion 25 mins in Clinic- rest with Ambulance transport – billable?

Thoughts?
Patient seen in clinic with saline infusion started. Time of 25 mins of infusion provided – with continuation of this during ambulance ride to hospital.
Billable on the clinic side? Thanks!!

Nursing documentation :
A #20 gauge catheter was started in his left forearm. There was a good blood return. 250 cc’s of 0.9 Normal Saline was then started per pump and infused over 1 hours. The patient tolerated the treatment well. Start time 1315. Stop time 1340.

The pt was transported to hospital as a direct admit via ambulance. The EMTs took the IV bag of 0.9 Normal Saline with the pt to finish giving it to him in the ambulance.

Two E/M Services, Same Day at Same Clinic, But Two Providers? What modifier??

Hello! So I work for a clinic, where we have a bunch of different providers who do different things, but they are all under E/M codes. So I just ran into something that I have never seen before. I was entering a 99215 encounter for a provider, and I saw that our EMR system automatically put it on hold because there was a 99214 billed the same day but for a different provider. I cannot use -25 since it is a different provider, and I cannot use -59 since it cannot be attached to an E/M code. I read through every modifier and none will work. In this situation, does the E/M code need a modifier? And if so, which one should I use??

Clinic / Specialist / Hospital / Confused….

A Cancer specialty clinic sees patients for care/treatment for Cancer at their clinic.
These same patients often have other comorbidities where they are treated in the ER and or Inpatient stays at the hospital located on the same campus.
Scenario:
-The Clinic Physician, who is not a hospitalist, treats a cancer patient at their (outpatient) clinic.
-The same patient ends up being admitted to the hospital for an unrelated condition.
-Because of the Cancer/Diagnosis the pt. has, the hospital staff physicians are not specialized to treat these patients.
-Due to this, our Clinic Physician will be called to manage the patient’s cancer while they are inpatient at the hospital.
-Our Clinic Physician will dictate progress notes and sometimes a Discharge summary for the patient.
-Our Clinic Physician’s progress note(s)/discharge summary will sometimes be directed at care of the pt’s cancer. Other times, the Clinic Physician will treat the (non cancer related) current condition(s) the pt. has.
I’ve looked at the guidelines but I’m a bit confused on this.
Anyone care to comment?

Suboxin Clinic

Okay I’m having trouble getting 80307 to pay I spoke to Aetna Better Health and they told me the mod QW i have on there is wrong but said it does need a different modifier does anyone know what modifier would need to be used to get this paid? I’ve tried to research this extensively and I can’t figure this out I’m pleading for help please!!!!!! This provider is based out of Kentucky.
