Coding resolved conditions ALONG WITH current conditions

I know that follow-up visits for resolved conditions are to be coded with the follow-up code (typically Z09) and then the history code for the now-resolved condition.

My question is:

How do you code / sequence codes for a visit that includes BOTH resolved AND ongoing and/or chronic conditions?

Here are a few examples of what I mean, where I am unsure of how you would code / sequence these…


  1. Patient is seen for:
    B/l foot pain due to an injury
    Sleep disorder due to shift work

    The sleep disorder has resolved because the patient switched their shift at work, but the foot pain continues as the injury is still healing.

  2. Patient is seen for:
    URI

    All URI symptoms have resolved. Patient mentions dysuria and is given antibiotics for UTI.

  3. Patient is seen for:
    Final wound check from laceration
    Hypertension (already diagnosed – chronic)

    Wound is completely healed. Hypertension is stable.

I am not having any luck finding guidance on this, so I really appreciate any help you can give!! Thank you!!

Healthcare News: CMS ICD-10-CM resource covers numerous specialties and conditions

CMS recently released an ICD-10-CM resource for specialties and specific conditions and services that collects varied educational tools, including webcasts, case studies, and clinical concept guides.
 
The clinical concept guides, offered for broad topics such as cardiology and orthopedics, include common codes for the related specialty, a primer for clinical documentation, and clinical scenarios.  

 

The resource includes links to all of CMS’ interactive case studies, which provide a medical case and ask specific documentation and coding questions. Upon completion, coders will be able to see which responses other coders chose, as well as sample coding for the scenarios in ICD-9-CM and ICD-10-CM.

25 yr old female patient with metastatic biliary cancer admitted to inpatient for neoplasm related pain and after 10 days of admission patient suddenly developed severe vaginal bleeding and gyn consultation was done, ordered labs . Labs shows high BHCG levels 1,6770 IU/L. Gyn consultant documents —found to have pregnant and underwent miscarriage. how would i code this encounter ?

Patient is 13 weeks pregnant admitted for missed abortion and underwent D&C, she also has ovarian cyst, asthma etc.would I code the other conditions to pregnancy related first trimester? Any official reference or coding clinic for this please suggest me ?

Other Auditiors opinions on HPI status of Chronic conditions

Hello,

I was doing a nursing home audit for two providers and there was differences of opinion as to what qualifies as a status of the chronic conditions. There documentation in the HPI is the patient is being seen for their monthly visit. Chronic problems being followed are diabetes, HTN, Hyperlipidemia, dementia, and Parkinson’s disease. Nursing reports no new questions or concerns at this time. There have been no new interval complaints from last seen. They are saying when the document "no new interval complaints" that is the status of the patient’s chronic conditions. To me that does not seem like a status–it is they have not had any acute issues since last time. Maybe I am thinking wrong???:confused:

Any help would be appreciated!!
