Patient with breast cancer in active treatment with ARIMIDEX with complaints of weight gain and mild hot flashes related to the endocrine therapy. In the assessment and plan my provider documents: stage I stable disease, tolerating treatment satisfactorily. ANED, normal exam, doing well, interval 6 months. We had a long conversation regarding endocrine therapy and the importance of watching diet. Counseling was provided regarding strategies to reduce hot flashes related to endocrine therapy including changing the schedule the medications are taken on.

Would you code for the weight gain and hot flashes if you’re not managing the patient’s medication?

Here is what I have so far:

1) breast cancer code

Is there a diagnosis I could use for this visit?

I know to code symptoms when the doctor doesn’t state a diagnosis but in this case I don’t even think there are any symptoms to code. What would be an appropriate diagnosis code for this visit?

Patient is a 6 month old.

HPI
Mom concerned about possible allergies
Eczema was flaring recently, mom thought it was from being in grass
Would get big welts after being in grass, welts on areas that didn’t even touch the grass
Back to back colds, cough hasn’t cleared for past 3 months

Doctor’s impression was:
discussed normal viral exposure during daycare ages, is growing well, no other physical sx, will watch and call for new or worsening sx
