CMS Identifies E/M Codes With High Error Rates

Coding initial hospital care became more challenging after Medicare stopped paying for inpatient consult codes several years ago — but that can’t be the only factor driving the startling error rates for evaluation and management (E/M) codes 99223 and 99214. In the report “2019 Medicare Fee-for-Service Supplemental Improper Payment Data,” the Centers for Medicare & […]

Clinical diagnostic laboratories that fulfilled orders for targeted genomic sequence analysis panel CPT code 81455 between March 16, 2018, and March 31, 2018, may need to resubmit these claims to Medicare. venipuncture-redraw after error

Hello All,

Looking for some guidance on the legality of the following situation: A patient comes in for lab work and the physician or phlebotomist missed something on the order and the patient has to come back. Do you no charge the venipuncture when the patient comes back as a courtesy because it was our error? Does the venipuncture go on the claim with $ 0.00 or can you leave it off completely? Do we bill the patient because the service was completed?

Thanks
Steph Mescher

G31.84 Mild Cognitive disorder use additional code error

I contacted AAPC back in August about the incorrect information included as reference under ICD-10 code G31.84. The error revolves around the use of additional ICD-10 codes F02.80 and F02.81. Both ICD-10 codes are listed under the Excludes 1 list under dementia (F01.-, F02.-, F03). Could someone look into this?

2016 study guide error

This may seem a little late, but I was reviewing for the 2018 CCVTC with the 2016 version of the study guide (I bought it years ago with intent on taking test sooner). I noticed a mistake with the case regarding mitral valve reconstruction. The answer given for the question regarding CPT code selection clearly is incorrect. I was wondering if this was corrected in later editions and/or is this something I should pay attention to on the exam (as the authors of the exam wrote the study guide).
