Your Chapter is Paid for Holding Meetings and Hosting Exams

AAPC pays $ 2 for each member-attendee at approved meetings and $ 10 for every examinee. No reimbursement is paid for seminars. This reimbursement is deposited into the chapter checking account on a quarterly basis.

No Copays for Eye exams related to Diabetes?

I recently had a payer Regence Medavantage plan call me and say that we should not charge copays for diabetic eye exams. I can’t seem to find any ruling about this from Medicare or the payer.

Anyone hear of this? Does the dr need to be treating or monitoring the pt for Diabetes?

Preventive Exams with Medicare Advantage Plans

Can anyone please assist me with proper coding for a Preventive Exam for Medicare Advantage/Medicare +Blue plans?

I am being told that a preventive visit is covered, as in 99396 or 99397 but I cannot get claims to scrub with these codes. Just recently I have started getting rejections and / or patients being billed for these visits. Plan states there is a preventive code but they wouldn’t give it to me.

Adding Screens to surveillance exams?

Several patients have contacted our office after being told by their insurance that we should be adding a screening code of Z12.11 for full payment of a SURVEILLANCE exam for personal hx of polyps (Z86.010). This is clearly not a screening. We have been informed that a very large practice has been billing with the screening code in the primary position resulting in full payment and no transfers to patients’ deductibles/co-insurances. We have always believed that we only code for the indication, as screens are only performed every 10 years and surveillance exams are more often based on doctor recommendation. Does anyone else charge for both screening(Z12.11) and hx of polyps(Z86.010) together? Any help would be appreciated.

Practice Assessment Exams CPC or CRC…are there any out there?

I took my CPC exam in November and my CRC in March I passed both with 82-84%, in my eyes not too bad. I just took a very short HCC assessment exam, 11 questions got 6 out of 11 right. Needless to say I am bummed, I want to improve my coding skills and want to practice so I do better next time. My biggest fear is the ole if you don’t use it you lose it!! I miss having my nose in my coding books both HCC and CPT coding. I am looking at the Practicode however I have heard it is flawed so waiting to see if it goes on sale as it did last year, don’t want to pay full price for a product that may not be completely up to par, sorry just my honest opinion.

I know it is hard for a new coder to break in to the field just want to find a way to keep the dust from settling on my skills that I have worked so hard to get.

Preventive Exams Medicare patients

We have a situation where "preventive health care" and "preventive examinations" are scheduled for Medicare patients. Medicare denies payment, of course, and we are asked to code the visit to an established E&M code, because the Medicare IPPE and Annual Wellness Exam components are not met.

I may be thinking way too much, but when I see verbiage as [presents for] "preventive health care"; "patient here for check-up"; presents for "physical examination" my first thought is to be wary on changing the CPT code from a preventive to a problem-driven E&M code.

The documentation has minor problems notated in the A&P, all established problems. To me, these established problems, would not warrant a problem-driven E&M code, as I feel they are minor components and would be bundled with the preventive exam codes.

I’m stuck here. Does anyone have any suggestions on this? We are working to educate the providers and staff, which is a positive step! But I don’t feel these visits should be changed. Is there any acceptable reasons that the visit can be changed from a preventive code to a problem-driven CPT code?
