Facet Joint Injection w/aspiration

I have not ever been asked this before and not sure myself. My pain doc performed a Right Facet Joint Injection (64493, 64494), he also performed an aspiration on the same joints. Can the aspiration be billed separately from the injection? Insert from the procedure report is below:

I advanced the needle towards the facet joint until I got good position of the needle into the AP, lateral and oblique view at the above levels. We had positive aspirate of yellow serous fluid from both joints and negative parethesia. I injected 1 ml of Omnipaque 300mg/mL, which showed good spread of the dye along the facet joint at the right L4-L5 and L3-L4. I had negative aspirate and negative paresthesia.

Facet Block Question

Need help guys,

How would you code the following:

1. Bilateral L5 dorsal ramus nerve branch blocks under fluoroscopic guidance with radiologic interpretation.
2. Right L4 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation.
3. Bilateral L3 medial nerve branch blocks under fluoroscopic guidance with radiologic interpretation.

These are all done on the same patient. I would very much appreciate any input.

Lipogem injection into facet joint

Hello,
We have an ASC that is injecting Lipogem or A2M into their facet joints. The providers want to use CPT code 64493, this code states a therapeutic or diagnostic agent is used. They are being used as a therapeutic agent. Per Medicare LCDs it specifies a steroid or anesthetic used but I cannot find anything from AMA that it specifies it has to be those substances. Does anyone else do these and what codes to you use for non Medicare payers?

HCC question on Facet Arthritis of lumbar

Our Pain management physicians love to code M46.96 for Facet lumbar arthritis of spine. After doing research the physician has to document inflammatory arthritis in order for the code to be correct. The imaging stated the arthritis is degenerative or osteoarthritis which will code to M47.816. Do anyone know if there is an article or coding clinic to help resolve this problem? Do anyone have suggestions?

facet injection with nerve branch block

Hello 😉

I have not done a ton of pain management coding, so any helpful tips would be very welcome! Today I have a specific question. My doctor states he did facet injection at L4-L5 & L5-S1, he also did L4 & L5 median nerve branch blocks. These were all done on the left. Would this be 64493, 64494 only? Or also 64495? or am I totally off? Thank you for your help!
