Physical therapy insurances that only pay for evaluation on first visit

I’m having a hard time trying to find information about which insurances will only pay for the evaluation code for the patient’s first visit with physical therapy. Does anyone know what the insurances are or where I can find that information? Thank you!

We are in the process of moving to Vancouver, WA- I have 15+ years of Medical Assistant experience seeking a medical coding job. I have passed my CPC test and was able to drop the A in Apprentice. CPC with lot’s of medical office experience looking for first “official” coding job!

Hi, I am located in Louisville, KY and I just recently obtained my CPC after working in insurance verification for 3 years. I have been employed at my current job for 6 years and have worked everything from medical records, front desk, billing, etc. I have not had an official coding job title but I have worked with CPT and ICD-10 codes quite a bit. I have learned a lot here and am ready to move on to a coding position. I learn things very quickly and work very efficiently. I will not disappoint! I can provide excellent references!

Need diagnosis code for labs BEFORE first psych evaluation/first visit

What is the correct diagnosis code to use when ordering screening labs to be done before the patient is seen the first time for a psychiatric evaluation? This is required at my facility. Currently my facility is using Z79.899 (Long term current drug therapy) but I believe this is wrong because the patient is not yet on medication. I have exhausted my resources so I’m hoping someone else does this and can help me.

First OB appt late in pregnancy

I have a claim that is denying from Medicare, our doctor billed a 99213 with dx Z32.01, O99.214, Z3A.36 and Z34.03. According to the progress note this is the patient’s first visit to an OB and she 37 weeks. I am very new to OB coding (like this is my first ever OB claim). Can anyone give me insight on how to handle this issue? Thanks so much for any help!

Can you bill e/m with fracture care for first visit to Ortho?

Patient was seen in ER and splint was applied. Next day the patient came to the Ortho office as a new patient where a new splint was applied, xrays reviewed, care instructions given and follow up apt made. Doctor billed 26600 fracture care code, can we bill for an e/m with 57 as well since patient was new and decision for fracture care was made at that time? And if there’s any credible sources/articles I can reference? Thank you in advance!
