Billing Office Visit + other and then only getting reimbursed the smaller amount

I’m trying to bill a 99213 along with an 81000. I’m getting messages from the carrier that they’re paying for the 81000 and bundling the 99213 under it (so I’m being reimbursed less than $ 10 for a service over $ 100). Is providing a modifier 25 for 81000 a way to get around this?

Thank you!!

Getting Through an Operative Report, Without Crying

One of the things I love about the mentoring I do for coding students is it reminds me of what it was like to be a newbie. And I don’t just mean the excitement of being on the cusp of a new coding career. I am also grateful to be humbled and reminded that I […]

HELP! using modifier 62 and 80 on the same claim and getting denials

Can anyone give feedback or help me find documentation on billing co-surgeon and assist on the same claim. In Appendix A of the AMA CPT book, modifier 62 states if a co-surgeon acts as an assistant in the performance of additional procedures, other than those reported with modifier 62, during the same surgical session, those services may be reported using separate procedure codes with modifier 80, as appropriate. If we are asked by another specialty to act as co-surgeon we of course bill with modifier 62 on primary procedures however since modifier 62 cannot be appended to instrumentation codes we bill with 80 on instrumentation. We are getting denials now from Horizon and Medicare on the instrumentation codes stating no qualifying base code is being used due to the the primary procedure being billed with 62 makes the TOS 2 and 80 makes TOS 8.
Example:
22551.62
22845.80
22552.62
20930.80

Thanks in advance.

Finally getting EHR now which one?

Our small Orthopedic surgery clinic is finally taking the plunge in 2019! So excited. So now we are trying to find the right software for us. We have 8 Surgeons each with their own sub specialty. We offer clinics in three locations, and operate in three different facilities. Our doctors currently rely heavily on dictation. I am drowning in possibilities so any recommendations would be welcome!

Thank you!

Getting history information

When reviewing a patients family and social history and there is no qualifying or pertinent information related to the HPI or chief complaint, but it is documented by the provider that they were discussed, if all other areas of comprehensive history are discussed, will those qualify the PFSH as complete, allowing for a comprehensive history?

Getting First Practice Manager position

Hello all,

Hope you can offer some assistance.

I have 20 years of senior level management/leadership, managing 90+ with 12 direct reports and $ 5 million P & L. Last fall I took the CPPM exam. Happily I passed the CPPM exam on the first try.

It’s my hope to transition to Medical Practice Management from my current leadership position.

Here is my question. I’ve had some excellent interviews. However, my lack of experience as Physician Practice Manger (PPM) is getting in the way. How did those of you in PPM get your start? There are 100’s of openings, most stating they want 2, 3 or 5 years experience. Seems there must adequate supply of experienced managers, as it is so often a requirement.

Thoughts??

Thanks!
