Diagnostic sampling of parathyroid hormone to manage calcium during parthyroidectomy?

Hi,

Do any of you bill CPT 36500 selective venous catheterization during parathyroidectomy, for venipuncture for diagnostic sampling of parathyroid hormone to manage calcium levels perioperatively?

We have told our docs their documentation does not support catheterization, so we do not bill this CPT. Documentation is describing venipuncture (no catheter placed), which is not payable under the physician fee schedule.

They argue their associations instruct them to bill this code.

We are wondering if the rest of you have experience with this and what information you might offer.

Well woman exam with hormone therapy

Hello,

52 years old Established Patient come for Gynecological Exam
HPI: Patient doing well except she is getting some flushes and flashes on the lower dose of estrogen

& for A/P : Well woman exam, Menopausal woman, On HRT with now some symptoms, once to evaluate the doses
"Discussed options with hormone therapy versus decreasing dose versus nothing versus how long she can take it and risks. I discussed the risks, benefits and alternatives to hormone replacement therapy with Heather. We discussed the results of the Women’s Health Initiative and the guidelines for HRT from ACOG. Some of the risks include stroke, clot, dvt, PE and she understands these risks and wants to take HRT". And Medication ordered: progesterone (PROMETRIUM) 200 mg capsule, Estradiol (DIVIGEL) 0.5 mg/0.5 gram (0.1 %) Gel in Packet.

Can we code extra 99213 with 99396?
Hormones review is included annually WWE?

Z79.890 Hormone Replacement Therapy(Postmenopausal) Gender Issue

I work for an insurance company and I’m seeing this code submitted on claims for males. Any thoughts as to why? Poor coding and misuse? Used for conditions other than postmenopausal like testosterone depletion? Let me know your thoughts why this would be justified for males.

My Doctors are wanting to allow an RN we have on staff to do pellet insertions which is in the scope of practice for TN. My question is, what are the billing ramifications of this? Would this be billed in a different way than if it were done by the Doctor? Thanks in advance for any help!

hormone labs denied by BCBS

Hi, Does anyone happen to know the Lab code for comprehensive hormone panel? We had lab charges denied by BCBS stating there is a combo code but I have no clue and would appreciate if someone could help or steer me in the right direction. Codes 80050, 80061, 82306, 82670, 84144, 84402, 84403 were billed to BCBS. 80050 & 80061 were paid and the rest of the codes denied.
