modifier 25 for e/m visit with immunizations?

Hi, I am new to the coding world and am struggling with applying modifiers. I have a few providers that insist on using a modifier 25 to report a preventative visit with immunizations. I don’t feel this is needed but they think I am wrong. Can anyone clarify this for me? Am I misunderstanding the definition in the book? Thanks for any help you can give me.

Does Z23 need to be the diagnosis for ALL immunizations?

Good morning,

I’m looking for some insight, I have a provider who wants me to code S81.812A-Laceration of left leg as a primary diagnosis for a TD shot. I was under the impression that all vaccines should be coded with Z23? Am I able to code the TD shot 90471 and 90714 with a laceration code as the diagnosis?

Can well child visits be billed without administering immunizations?

I work for an FQHC. We no longer have VFC’s for immunizations. Can we bill for well child visits without doing the immunizations? We were trying to see if we can perform the well child visit and send the patients to the health department for immunizations? Please Help!

PP care after delivery and immunizations and flu during pregnancy

Patient is seen for PP care after deliver. We perform a 87210 and our coding department entered N89.8 and ties this to 87210 but then enters Z48.816 as another DX on the claim. Should the Z48.816 also be linked to 87210 or not? Or should it just be listed on the claim or not listed at all?

Patient is 28 weeks pregnancy and a flu shot is given to patient. Does the weeks of gestation need to be added to claimZ3A.28? 90686 and 90471 with Z23.
Or would this be a third party of Medicaid carrier discretion?

TB Skin tests and immunizations billing

If you have a clinic that is sending their employees off site to provide employee health services such as TB skin tests and flu shots for an outside company, do you still need a ‘physician’s order’ to satisfy billing that to insurance? Or is it ok for a nurse to administer w/out a physician signing off on an order but still bill insurance?

Immunizations pre-splenectomy

Hello all. I’m cross-posting this from the General forum due to lack of response there.

We have a patient who will be having a splenectomy in the near future and her doctor wants her to have meningococcal and Hib immunizations prior to surgery. Patient has no known exposure but is high risk due to her blood disorder and soon-to-be absence of a spleen.

I feel this is preventative use and we need only use ICD code Z23 to report the immunizations but want another opinion. Has anyone else dealt with this? Did you use Z23 or did you report the disease making the patient high-risk for infection?
