IS it appropriate to bill a dialysis run (90935) and a subsequent E/M for the same specialty on the same day for a inpatient stay (99231 – 99233) with a modifier? Is there any verification out there besides the cpt book?

inpt chemo treatment
I’m newer to coding oncology, I was trained briefly before the last person covering the division left. If a patient is admitted to the hospital for chemo treatment and they did the treatment and the doctor noted that they completed their course of chemo treatment on a certain date…the primary diagnosis for those days of chemo treatment are Z51.11…NOW the patient stays in the hospital for monitoring and count recovery for several days after do I still code the Z51.11 as primary diagnosis for all those days even though she is no longer having chemo treatment but it’s related to that OR do I now just code for the cancer as primary and any additional diagnosis?
obs to inpt
pt is adm to obs stays in obs for 2 days and then becomes inpatient
hosp facesheet ex:obs may 4 changed to adm may 6 th
do we bill obs for may 4&5
do we bill inpt for may 6th and would it be a 99221-99223 or 99231-99233.
bs will not pay for may 4&5 when bill as inpt because hosp adm facesheet states adm may 6th.
I can’t remember, I am thinkng I can not bill 99213 after we sent to ER and in/pt ad
I am having a memory blockage. We had a pt come in(work in) and my doctor decided to send to ER (at our smaller county hospital), patient had more than they could handle and sent pt to bigger hospital and was then admitted for an IN Pt stay now being seen by internist and oncology. Am I able to bill for the office visit 99213. Our office note and the local ER doctor both say PCP sent pt to hospital.
CPT or I10-PCS for inpt surgery?
I’m wondering if my urology practice should be using I10-PCS procedure codes instead of CPT when they are doing a surgery over at the hospital and the patient is in an inpatient status? Or the CPT procedure codes b/c we are a clinic and our Dr’s NPI numbers are connected to the clinic-they are not employees of the hospital.
