Place of Service – For Independent Labs

Dear All,

Would you happen to know what the place of service should be for the services done at an independent lab? The Medicare claims processing manual states it should be the location that draws the testing specimen. However from my personal experience I’ve seen a lot of laboratories bill the place of service as 81- independent lab although the specimen was drawn at a clini/physician’s office (POS -11).

Appreciate any thought on this.

Extended drug screen labs PLEASE HELP

If anyone can help me or answer this question. We have a psychiatrist that has been ordering extended drug screen panels and she is marking it P1252 and that is not a code I know it has to be lab based not CPT based.

One I need to know what that code is used for if anyone knows and two I need to know if we are clia waived if this is a blood test we can do in house or if it has to be done in a lab setting.

Any answers are appreciated. My chief operating officer wants me to figure this out this morning so any help is welcome

Diagnoses for Baseline Labs prior to beginning bioloigics

Hello All,
I am fairly new to Dermatology coding. We do a lot of treatment and prescribing of biologics for psoriasis. These are powerful drugs that can affect the organ systems negatively; therefore baseline laboratory tests are required prior to starting the patient on these medications. AFTER the patient has been on these meds, we continue to run lab tests to monitor if/how these medications are affecting the organ systems. Z79.899 usually works for monitoring (there are a few NCD/LCDs that do not cover with this code), but I’m wondering what ICD-10 code should be used for the initial baseline lab testing? We are not monitoring the patient’s response to the medicine since they aren’t on it yet, but simply getting a baseline. Many of these lab tests have NCDs and we’re having trouble finding appropriate diagnosis codes? Thoughts anyone?

Denial for Labs

My claim is being rejected when I try to submit it with 80053, 80061, 84443, and 85025 for a patient who is 17 years old. It states "Procedure code is not typical for a patient whose age is 17 years. The typical age range for this procedure is 18 years – any. I have also tried using the bundled code 80050 and it also came back rejected. Insurance is UHC.

Can someone help me with this please?

what ICD 10 codes that can be used for preventative labs. Please help!

when patient comes for a yearly preventative care visit, my doctor order CBC with diff, CMP and TSH ( general health panel CPT 80050). I use ICD 10 codes : z13.0, Z13.29 to bill for these labs but BCBS does not pay. How should i bill for these labs? Please advise me what ICD 10 codes should i use for these labs. Thank you.
