Mammograms

We are doing mammograms in our office now and I had a questions regarding diagnostic 3D mammograms. Someone stated to our Mammo manager that we should not be billing a call back diagnostic 3D mammogram if we already performed a 3D screening mammogram. I have been billing 2D & 3D diagnostic mammograms after a 3D screening was performed and all the insurance companies have been paying for them. Is anyone aware of this being incorrect?

I understand that we bill for both diagnostic and screening mammograms on the same day.

77067 – screening mammogram
77063 – 3d tomo

77066 – diagnostic mammogram
g0279 – 3d tomo

Here is what I am coding for – 77067 & 77063 for the screening mammograms (59 modifier on each) and 77066 and g0279 for the diagnostic mammogram (GG on each for Medicare claims only)

We are getting denials for the g0279.

Can we bill tomo twice per day if its used for a screening mammogram and a diagnostic mammogram?

-GG Mod with Medicare Mammograms

I bill for the professional side of radiology, and we have only had one or two patients this year have a screen and diagnostic mammo on the same day. So far we have had a hard time getting these paid. We are putting the -GG on the diagnostic mammogram, for one patient in particular, this is what we have billed out:
77067-26
77063-26
77065-26-GG
76642-26
The only thing medicare paid on the claim was the 77065 and 76642, they completely denied the screening and tomo.

I’m thinking I have read too much now and I’m just thinking too hard, can someone spell it out simply for me? Or point me in the direction of valid info from CMS?
