Morbid obesity – E66.01, (HCC), 50 minutes spent with the patient, with over 50% of that spent on discussion of morbid obesity, her attempts at weight loss, the risks and benefits of bariatric surgery. Patient would like to proceed with evaluation.
This was outside of the patients AW visit of 30 minutes
The excludes note for 99404 indicates not allowed with her Physical 99395
The 50 minutes fall short of a prolonged service code
Would it be appropriate to add an E/M for weight loss counsel consideration for Bariatric surgery during her Wellness visit?

Morbid Obesity with documented BMI of 34.9 or below

“Morbid Obesity” has always been supported by BMI values of 40 or more or 35-39.9 with co-morbid conditions and “Overweight” has always been supported by BMI values of 30-34.99 supports.

In ICD 10, when coding from category E66, there is an instructional note to use additional code to identify the BMI, if known.

Is it appropriate to report E66.01 (Morbid Obesity) with a documented BMI of 34.0? Does a BMI of 34.9 and below support the definition of morbid obesity?

I have some questions about G0447. Any resources besides Medicare’s website?

1) Documentation requirements for billing code? Time documentation required?

2) Can it be billed with an e/m? Other preventive visit?

3) Can you bill if they only discuss with the patient at one visit? Or do you have to have more visits?
