Billing patient when referral not obtained

My understanding (correct me if I’m wrong) is that if our office fails to obtain a required referral, we have to write off the visit; we can’t bill the patient.

But what about a situation in which the patient requests to be seen right away without waiting for the referral, and states that if we don’t get it, he will pay out of pocket?

Appeal for wrong place of service auth that PCP obtained

I work for an ASC and the PCP obtained an authrozation for a different Surgery center( which we did not know about ). We did the surgery submitted charges with auth number the PCP provided, and we were denied due to no prior auth obtained for our surgery center. pcp tried to change it to our facility but ins company will not change and told PCP, we will have to appeal it as the ASC. I can’t figure out how to fight this.

CPT 93922 No abi obtained, not billable?

Hello,

A provider is billing CPT 93926 duplex scan lower extremity limited or unliat lower extremity study and CPt 93922 lower bilat abi study, would it be correct to remove the charge for CPT 93922 –

For CPT 93922 when the report states non-compressible distal vessels prevents abi Reporting, right sharp monophasic flow at the ankle, lower extremity imaging not performed. Left no velocity increases found imaged lower extremity arteries. Sharp monophasic flow at the dorsalis pedis and peroneal at the ankle; broad monophasic flow at the posterior tibial at the ankle. There are findings of the left femoral through peroneal arteries values documented.

I feel I should only bill CPT 93926, any advice would be greatly appreciated.
