Colonoscopy CPT will be billed as G0105 with Dx: Z86.010. Medicare started deny claim " This is a non-covered service because it is a routine/preventative procedure" ??? Anyone have the same denials for Medicare patients in Florida? I checked LCDs for Medicare (First Coast Service Options) in Florida but I don’t find any changes. Please advised, Thank you so much.

Treament options or number of diagnosis
Just for clarification, if a patient with no symptoms presents for a consult for screening colonoscopy, would this be scored as self-limited or minor, or new problem additional work-up planned. There is some disagreement in our office about this. Or if anyone could point me to a source to be able to have concrete rules about screening consults that would be great too. Thank you!

Hello everyone,
One of our providers is seeing a new patient with several chronic conditions. He is managing three of these conditions (all problems new to him), however I am struggling with the Marshfield Clinic (MC) tool.
According to the MC tool, there is a Max=1 for New Problem without any additional workup done. Because the provider is managing several chronic conditions, it seems a bit unfair that he is getting penalized for the Max=1 and not able to get his 4 points in the Diagnosis and Management Options. I realize this might be where the MC tool is imperfect.
I can place one of the chronic conditions (fibromyalgia) in the New Problem without any additional workup, however I am puzzled as to where I can put the other 2 (chest pain, hypermobility). Since neither of the two problems are Established problems, I can’t really put them there; however both might be too severe to be categorized as a "Self Limited/Minor" problem?
Any help/guidance would be greatly appreciated!

Would visits with level 4 detailed exams and 3 diagnoses under Amount/complexity of data be all level 4? with key factor being at least 3 dx listed in assessment? Assuming table of risk is moderate/level 4 (Prescription drug Management) also assuming 1 or less points under Amount and complexity of data (low or minimal)
