Using our xray machine to do outside xrays

I work for an orthopaedic group and we have a brand new x-ray machine. The PCP on the second floor of our building would like to refer his x-rays down to our office for my staff to take the x-ray, my physician to read the x-ray and dictate a report and then bill for the x-ray – even though they will never see or treat the patient themselves. Does anyone have any knowledge of how this would work or if it is even legal?

placenta specimen from outside facility

Hi fellow coders
Please help with a coding issue. We have an attending physician that requested we get the placenta from the birth hospital for an infant being treated at our facility. The placenta arrived along with slides (we did not request the slides, just the placenta) and their report. We grossed the placenta and the pathologist will review the outside slides along with the slides we processed. Do I code this as an outside consult even though the outside facility did not request the consult (88323)or level V specimen (88307)? Thanks

90696 for patients outside of 4-6 years old

Hello,

The nurse at our clinic who administers vaccines sometimes admins a 90696 for kids outside of the 4-6 age rage. She says it’s due to patients being behind on their vaccines and circumstances like that.

Can I code this a a DTaP 90700 and IPV 90713 separately? Or do I have to use the 90696?

I’m the only certified coder and I’m new myself, so any help would be greatly appreciated.

Pathology Consultation of outside slides 88321

Discussion among the pathologists is 88321 x1 for the patient OR 88321 for each surgical case submitted. Can someone help clarify this?

EX: Patient A – slides submitted from outside facility: endometrial case from 2017, hysterectomy from 2017, a second endometrial later in 2017 and vaginal biopsy 2018. Some of us say 88321 x1 but a some say we should be able to do 88321 x4 for the different surgical cases. 😮

Diabetic counseling for OBs – payment outside the global package?

I’ve researched this quite a bit on the internet and AAPC forum. I’m hoping to find out if there is an OBGYN clinic who has been successful at receiving additional reimbursement for Diabetic counseling to an OB patient. Our OBGYN clinic would like to institute diabetic counseling to diabetic patients with the NP who is part of the OBGYN clinic. This would not be a service as part of their routine OB care, and would be scheduled specifically for counseling. I’ve seen options of waiting until the patient delivers and then billing an E&M if they exceed the routine 13 visits. I’ve also seen options for billing G0108 if 30 minutes of counseling is provided and billing that at the time of service. Another suggestion was bill and E&M with a -25 at the time of service. Anyone have success in billing one of these methods out of the OBGYN clinic?

Bill diagnostic test for TC alone from outside referral?

We have outside physicians refer patients to our clinic for the sole purpose of using our diagnostic equipment. Can we bill for the TC of the test(s) performed? If so, a technician is the one that performs the service (with no interpretation), who would we list as the rendering provider on our claims? Any insight is appreciated!

CPC-A Cover Letter for Positions OUTSIDE of Coding

Greetings,

I just passed my exam this week (CPC-A, never worked in a medical office) and have a quick question regarding the focus of my resume/cover letter for positions OUTSIDE of coding in the revenue cycle (ex front desk, charge entry, patient registration, data entry, etc). My strategy is to emphasize my desire to gain experience elsewhere in the revenue cycle in order to gain a more well-rounded skillset with the ultimate goal of moving into coding. Would you recommend framing my application to these types of positions as a stepping stone to getting into the coding profession, or should I forget that and simply try to highlight my skill set as it relates to the position I am applying for? After all, my main selling point is a coding credential. Thanks a bunch!

Global or outside of global

The first visit with the provider was coded as a threatened abortion since that is what we knew at the end of the visit. The second visit was a missed AB. There were some ultrasounds and a few more follow up visits after that. The insurance requires Global billing but has put all the visits towards the patient’s medical benefits. I understand that most of this is insurance specific. I don’t feel like we can change the coding and if I did just a positive pregnancy test for the first visit it would still fall out of the global. I just wanted to make sure that my thinking is correct.
