Pediatric behavioral health coding educational materials

Hello everyone,

I work with a pediatric multi-specialty group that also has behavioral/mental health services via psychologist/psychiatrists and soon to also include LCSW in our Pediatric Cancer Center group as well as a Developmental group and genetic counseling. I have never had the opportunity to work with behavioral/mental health coding/billing before and am looking for some good resources to learn. One of my big items is deciding when it might be more appropriate to use the behavioral health codes versus E/M codes for our psychiatrists and psychologists. We outsource our actual coding reviews and having worked with them in the past, I’m concerned that we might need to review what the providers as well as the coders have been taught in the past so that we’re all in compliance and in sync with each other.

Anything you might be able to suggest would be greatly appreciated!

CDE in pediatric endocrinology clinic

Hello,

I don’t know why, but I posted the below message in the forum and it was not visible anywhere except under my profile for my posts, so I’m trying again…

I work for a multi-specialty pediatric group and our endocrinology group has hired several Certified Diabetes Educators who are also dual certified as Registered Dietitians. I am looking to find some reputable resources for guidance in billing for their services. Not all of our pediatric patients have a diagnosis of diabetes yet, but are starting to show signs and often also are overweight so our CDE/RD are doing nutritional counseling and I guess you could call it preventive based education/counseling on the risks of developing diabetes.

Any help would be appreciated!

Pediatric ECMO Coding for a circuit Change

Hello, my clinician and I are disagreeing on what CPT code should be used for an ECMO Circuit Change. I am leaning towards CPT 33949-Extracorporeal membrane oxygenation (ECMO/extracorporeal life support (ECLS) provided by physician; daily management, each day, veno-arterial.
Here is the scrubbed documentation; PROCEDURE: This procedure was done at the bedside in the NICU. We prepped and draped the cannula sites in the usual fashion. We prepared the venous and arterial lines from the new circuit and these were held in place in a sterile fashion. We then made incisions in the cannula at the connector sites. We then clamped the venous and arterial cannulae on the patient’s side and on the circuit side, therefore coming off ECMO and at that very moment, the patient was put on rescue settings on the conventional ventilator. We then disconnected the old ECMO circuit by detaching the circuit tubing from the venous and arterial cannnula and these were moved away. We then brought the new venous and arterial lines and attached them to the cannulas, ensuring that we did not introduce any air into the circuit. We then went back on ECMO with good flows and achieved an immediate improvement in the pressure gradient across the membrane oxygenator. I was present for the duration of the procedure. The circuit change took approximately 90 seconds.

When do pediatric codes become ‘personal history of’ for an adult? Is it 18 or 17?

Recent discussion on a denial for age specific diagnosis of T74.4XXS (Shaken syndrome) on a patient that had reached the age of 21. So, the question is at what age does the patient move from an active pediatric code to a ‘personal history of’ code? I can’t seem to find a reference any where on this specifically.
