Routine physical: 99211 or 99212?

New to E/M and need guidance. Established patient in for a routine physical.
Everything looked fine. Mention of two systems reviewed. Doesn’t need to
return for 12 mths.

Not sure if anything would count towards 212.
Still, I’ve seen an article while looking for answers that said only 3% of
cases are actually 211 so… Any help is appreciated.

Physical Exam Documentation help

I’m an internal auditor and I have run into some exam documentation that I have not encountered before. The physician is using "denies" within the exam section which to me is more like ROS documentation, not physical exam.
Musculoskeletal:
Wrists Denies swelling, denies tenderness
Ankles Denies swelling, denies tenderness
Feet Denies swelling, denies tenderness

I feel like this could be a potential issue with an outside auditor. Am I completely off base?:confused:
If not, would anyone be able to point me to information I could use to educate the physician?

97 Physical Exam bullet: extremities

Can someone help me know whether an exam bullet(s) for extremities can be counted in the example below.

EXTREMITIES: External fetal monitor shows a fetal heart rate baseline of 135 with minimal to moderate variability and spontaneous variable and late deceleration’s. External uterine monitor shows contractions randomly approximately every 10 minutes.

Can a bullet be counted for the uterus contractions? what about the fetal heart rate and late deceleration’s?

This is obviously a pregnant patient, this is taken from the hospital HPI.

Initial Medicare Physical

This question is in regard to the Initial Medicare Physical that patient’s can have within their first year of enrollment…

Part of the exam is a "simple visual acuity screen." My physician is wondering if the exam can be performed using the Jaeger Vision test cards rather than the wall chart. Any idea if these are acceptable?

Physical Exam Documentation

This is a question about documenting the physical exam.

Our Clinical treatment forms are templates that have "check" boxes for ROS, PFSH, Physical Exam Body Areas/Organ Systems. There are also diagrams of the body since we are Dermatology specialty.

When the provider is documenting the physical exam, can he/she just draw a line to the body part on the diagram and write what the finding is without checking the appropriate box for body area/organ system?
OR
does the provider have the check the appropriate box under the physical exam section indicating which body area or organ system was examined?

Any help is appreciated. Thank you.

Physical Therapy

Hello,

Physical Therapy is a new area for me and I was given a question from one of my members from the billing team about Denial from the Insurance Company. They billed for DOS: 05/11/2017 CPT codes 97110, 97116, G8978 DOS: 05/17/2017 97110, 97116, 97140, G8979, G8980. They are denying for Modifier missing & not properly paired G codes? Can someone please tell me where to start looking? Do you have a link? want to see if we can correct before the 1 year deadline.
