Billing CCM Care Plan Code (G0506) and ACP (99497/99498)

Hello!
We are currently having discussions around the appropriateness of billing the CCM Care Plan Code (G0506) with ACP codes (99497/99498). There is no NCCI edit that prevents these codes from being billed together, but it seems like a muddy area.

Our concerns are that the conversations and work that goes into billing the G0506 code often overlaps with the conversation and work that goes into billing ACP codes. But at the same time, during a CCM initiating visit, if a provider performs extensive assessment and CCM care planning, documents the status assessment and plan for each chronic condition, and also documents a conversation around ACP that reaches at least 16 minutes, would it be appropriate to bill for both the G0506 and 99497 codes?

Any insight/opinions on this topic would be much appreciated!

Medicare Advantage Plan (Humana)

Humana has been denying our Electrocardiogram, routine ECG with at least 12-leads; interpretation and report only; 93010. Our ER physicians bill this procedure code and Humana has been denying them stating the procedure code is incorrect/or invalid. Of course, I have call Humana and they only say I need to send in the ER notes. Then I found out from coding/editing at Humana that they feel as if the ER physicians abuse this CPT code this is why they want the ER notes. Does anyone know where I can locate the Medicare guidelines for this CPT code? Medicare does pay for this procedure. Or if anyone knows where I can find Humana’s policy about this CPT code it would be greatly appreciated.

CA Dx – WTC Twin Towers Medical Plan

Hello,
I have a patient who was involved in rescue efforts at the Twin Towers on 9/11. He subsequently developed cancer on his palate. The patient had surgery to remove the cancer, but this left a defect that needed surgery at a later date. That’s where our plastic surgeon came into the picture. He repaired the defect in the palate with the diagnosis codes K13.79 Other lesions of oral mucosa, K13.29 Other disturbances of oral epithelium, including tongue, Z42.8 Encounter for other plastic/reconstruction surgery following medical procedure or healed injury, Z85.09 Personal history of malignant neoplasm of digestive organs. This payer will not accept any diagnosis other than the C05.1 Malignant neoplasm of soft palat, however, the cancer is not present anymore. It is my understanding that once the cancer is removed it cannot be coded anymore.
