No Diagnosis for Point of Care tests in office

I am seeking some information on what others are doing or what should be done for this situation.

Physician will document for conditions that are addressed such as Neck Pain (M54.2) and Paresthesia (R20.2).
Then a Point of Care test will be done that does not have any diagnosis associated with it. For instance, note will have the two codes given then have a POC Hepatitis C Screen and POC HIV Screen done.

1. for these tests would you only put the M54.2 and R20.2 on these line items. Since this is exactly what the provider signed off on.
2. query the provider for a diagnosis for these tests.
3. coder add the screening codes for these tests.
4. other??

Trigger Point

We recently transferred from Cahaba Medicare to Palmetto Medicare. I have never had an issue with our trigger point injections (20552 & 20553), but since transferring to Palmetto, I have rec’d several denials stating not medical necessity. When checking the LCD for acceptable ICD 10 codes, there are not any listed. Any suggestions on how to find the ICD 10 codes that Palmetto will accept???

PVR – Post Void Residual — Imaging data point?

In urology they do a post void residual test 51798 where they have the patient empty their bladder then us US probe to measure the volume left over post voiding.

It’s not a 70000 code… but would you still give a review/order imaging data point for this??

Trigger point injections

I am having an issue with trigger point injections denials through Medicare. One of our providers does a fairly large number of these and I have usually coded 20553 with diagnosis of M62.830 (paraspinal muscle spasm) These have always been covered under Medicare but I am now starting to get denials for medical necessity. I have looked up the LDC but no approved dx are listed. Is anyone else having this same problem?

EM coding Data point?

Hi Everyone,
I have 2 questions and hope to get some ideas:
1. We are Pulmonary medicine clinic and do PFTs here which we bill for. When the doctors order a PFT during the visit, can they count this as a point toward their MDM (data point, review or order a test in medicine section of CPT)?
2. The doctors always review the imaging themselves and sometimes review the Radiologist’s report as well to compare. They get 2 data points for review and summarize the imaging, but do they get a point as well for reviewing the Radiologist’s report? With good documentation, do they get 3 data points?

Acquire Medical Digital Images With a Kodak Point of Care CR System

Small and mid-sized medical facilities, mammo centers, radiology departments, imaging centers, dental and veterinary offices can benefit from a cost-effective Kodak CR system for acquiring digital medical images.
