Unrelated Procedures or Visits leading up to the “Pre-Operative” Period

Scenario:
A member is inpt., provider is billing code 99232-24 (DOS 01-28-19) mod. 24 as member is still post-operative of another 90-day procedure…

Now day after (01-29-19) provider billed 99232-24-57 as they are seeing the member in post-op but also made same day decision for another 90-day procedure (61510-58).

How should the 01-28-19 DOS be billed as it is the day prior to decision of the major surgery and provider is indicating unrelated and decision was made the next day/same day 01-29-19?

01-28-19 99232-24 denied because the visit was billed by the same provider within the 61510’s pre-operative period.

ICD-10 Coding For Preoperative Clearance

I work for a Family Practice physician who often does preoperative clearances for local surgeons. I need a little bit of guidance on coding for for a recent visit. What ICD-10 code should I use if the surgeon requested that he do a preprocedural EKG (Z01.810), a preprocedural chest x-ray (Z01.811) and preprocedural labwork (Z01.812)? Can I use all three or should or should I use the "OTHER preprocedural exam" (Z01.818)? Many of his patients have multiple chronic issues. I have not seen this situation discussed previously. Any help is appreciated.

pre-operative visit within a global period for related procedure

If the patient had a partial removal of colon with colostomy and is still within the 90- day global period, can/should the same physician bill for the pre-operative visit prior to the colostomy reversal? It feels like this should just be a 99024 post op visit but another coder is telling me we MUST bill the pre-op with modifier 24?

Does anyone have a link to an article on this topic that they could share? I can’t seem to find any example like this….

any help would be greatly appreciated

Thanks

How to Code a Preoperative Clearance

A preoperative examination to clear the patient for surgery is part of the global surgical package, and should not be reported separately. You should report the appropriate ICD-10 code for preoperative clearance (i.e., Z01.810 – Z01.818) and the appropriate ICD-10 code for the condition that prompted surgery. A preoperative history and physician (H&P) is included in the […]
