58301 IUD removal rejection

Hi, everyone, good afternoon, does anyone knows why :confused:
58301 is the only code for IUD removal, am I right?

Wellcare rejected
99213(25)+N92.5—paid
58301+Z30.432—rejected, N657 This should be billed with the appropriate code for these services.

Any help and knowledge is appreciated, thank you very much.

CO 252 rejection code – what information are they lacking?

Hi everybody!

This is the first time I’m writing here. I have a strange claim that was denied with CO 252 code and the appeal wasn’t successful either. The clinical was attached but they still say that after consideration they don’t think that the visit is as complex as they need for 99205 (new patient). Here are they ICD-10s that were billed accordingly:
R10.84 Generalized abdominal pain
R11.2 Nausea with vomiting, unspecified
F41.9 Anxiety disorder, unspecified
F41.0 Panic disorder without agoraphobia

What am I doing wrong?

Medicare Payer Rejection

We can bill state Medicaid under our certified counselors but Medicare does not recognize them. I am trying to send to Medicare anyway to get the denial to send to Tricare as they said they would consider with the denial. However I cant even get the claim accepted thru our clearinghouse as Medicare rejects it for unknown NPI…any suggestions?

Peachstate(GA) rejection M81

Hello,

Can somebody please help me on resolving this rejection?
I have Peach State claims that CPT 99213/76856 are rejecting with M81 saying "not otherwise classified" or "unlisted" procedure code was billed when there is a specific procedure code for this procedure/service.
I have no problem with other claims with peach state with the same codes, but only a few of patients have rejections.
I have tried to change the ICD-10 and also changed 76830 to 76856, but still rejecting.
Please help!! :(

Rejection for 95819-26 billed with Consult in POS 22???

Can someone please tell me if I should be getting a rejection for billing a 99255-25 and 95819-26 together? It is an outpatient consult billed with the reading of an EEG. Not sure why, but I have 2 claims going out to AmeriHealth that keep rejecting stating that the POS is incorrect. PLEASE help, someone! I have checked a few different resources to see if they can be billed together in an OP setting (which I already know they can….unless something has changed) and every tool I use to scrub the claim accepts them without issue.
