Florida Physician reports a HIPAA Breach that could affect over 60K patients

Authorities at doctor administrations merchant EmCare in Florida have advised 60,000 patients, representatives and contractual workers about a cybersecurity occurrence that may have uncovered their own data. An unapproved individual had the capacity to get to various worker email accounts that contained clinical information and statistic data, alongside driver’s permit and Social Security quantities of certain people, as indicated by an examination.

Preliminary Radiology Reports included in a Final Report Record in the ED.

Hi There,

I am wondering if anyone could clarify the correct way to document a Final Report in the ED.
To my knowledge – especially when dealing with Medicare Claims; a Preliminary Radiology Report should not be a part of a Final ED Note. In other words – the Radiology Report should be finalized as well, when it is interventional and a part of the diagnosis decision making process. Am I correct in this thought or is it incorrect? Please enlighten me.

CMS making deficiency reports more accessible to the public

CMS wants to put a stronger spotlight on times when TJC and other AOs fail to find patient safety problems later discovered by CMS survey teams conducting performance checks referred to as validation surveys.

To do so, CMS has created a website that provides a graphic about the disparity rate of deficiencies found on validation surveys and then posts deficiency reports from hospitals it says have “recent substantial deficient practices.”
