One specimen, or two?

I’d like some opinions on how to count the specimen(s) in this case, based on how they wrote up the accession.

We’re having some arguments over this w the path dept on how to count them based on how they accessioned the specimens as a single specimen (submitted), yet the gross supports that the tubes were id’d separately and in separate cassettes. Most of the coders and audit managers think this is ONE unit, as the specimen is written up as a single specimen (not Spec A. tube L, Spec B. tube R) so it doesn’t qualify as separately submitted, despite the gross noting separate identification.

SPECIMEN:
FALLOPIAN TUBES, BILATERAL, TUBAL LIGATION WITH ID, RIGHT TIED

GROSS DESCRIPTION:
The specimen is received in one part labeled XX-19-xxxx; Txxxx, Xxxx and medical record number xxxx.

Received in formalin labeled "bilateral fallopian tube segments" are two tan-pink, cylindrical, grossly unremarkable fallopian tube segments, each measuring
2.0 cm in length x 0.5 cm in diameter. The right segment shows a portion of fimbria and is indicated with a suture. The specimen is serially sectioned and entirely submitted in two cassettes as follows: cassette 1 – right segment; cassette 2 – left segment.

placenta specimen from outside facility

Hi fellow coders
Please help with a coding issue. We have an attending physician that requested we get the placenta from the birth hospital for an infant being treated at our facility. The placenta arrived along with slides (we did not request the slides, just the placenta) and their report. We grossed the placenta and the pathologist will review the outside slides along with the slides we processed. Do I code this as an outside consult even though the outside facility did not request the consult (88323)or level V specimen (88307)? Thanks

Billing CPT 82542 multiple times for single specimen

Hi there, any help/assistance on the below question will be grateful:

While processing Lab & Pathology services, I find the provider billing CPT "82542" with multi-units for only 1 specimen(Urine). Whereas, multiple drugs/metabolites analysis was done. So the question is:

Can we bill multiple units for single specimen? As the code description says "Column chromatography, includes mass spectrometry, if performed (eg, HPLC, LC, LC/MS, LC/MS-MS, GC, GC/MS-MS, GC/MS, HPLC/MS), non-drug analyte(s) not elsewhere specified, qualitative or quantitative, EACH SPECIMEN"

Billing for a specimen that did not survive pro essing

If a specimen is embedded and a slide is cut but no tissue has survive upon tech examination, then confirmed by pathologist, we can bill for TC on say 88305 right!? If a special stain was also done prior to pathologist’s examination due to rule out ordered by referring physician are we able to bill for the TC on that stain or does it fall on the tech who processed the H&E to not stain due to no tissue after processing? I hope I’ve made myself clear lol.

I have a question about billing for multiple specimens collected on thyroid biopsy my boss& coworkers believe we should be able to bill for cpt 10022 if the specimens were collected from separate areas using a 59 modifier. Does anyone have knowledge about this?
