Revised guidelines for E&M When Performed with Superficial Radiation Treatment effective from January 1, 2019


CR11137 revises Chapter 13 of the Medicare Claims Processing Manual to allow providers to bill E/M codes 99211, 99212, and 99213 for Levels I through III, when performed with superficial radiation treatment delivery (up to 200 kV), when performed for the purpose of reporting physician work associated with,

  • Radiation therapy planning
  • Radiation treatment device construction
  • Radiation treatment management when performed on the same date of service as superficial radiation treatment delivery.

According to Current Procedural Terminology (CPT) guidance, providers should not report superficial radiation (up to 200 kV) with CPT codes for planning and management.

Billing of these E/M codes with modifier 25 may be necessary if National Correct Coding Initiative (NCCI) edits apply.

Coding Ahead

Neurosurgeon radiation treatment planning with a radiation oncologist physcian.

Hi All,

I am new to auditing and have a question. I appreciate any feed back I can get as well as reference materials (if possible).
I have a neurosurgeon who at times will resect a spinal metastatic tumor then partner up with the radiation oncologist to obtain a radiation treatment plan for the patient. He insists on billing 63620. However, my understanding is, if he has already performed the surgical resection the planning is already included in this code. Can this code be billed in this case?
Second scenario is if he does not perform a surgical resection, however, is part of the radiation planning, can he bill this code in this scenario. Again, I am very appreciative and grateful for any assistance/advice.

PENS treatment

We are having problems with getting payment on PENS treatments which is coded with 64555 it was paid on in the past has anyone been getting paid for it recently?
PENS stands for Percutaneous electrical nerve stimulation and really helped out patients out with their pain but we have not been able to offer it since it is not getting reimbursed? Any feedback would be greatly appreciated.

Medical Care at Psychiatric Residential Treatment Facility

We have a physician from our outpatient office travelling to our Psychiatric Residential Treatment Facility to provide non-mental health E/M services when requested. I am having a hard time defining The PRTF to select a code for this. I’ve tried to research what category this location would fall under and can’t find much.

Would I consider the PRTF to be a nursing facility?

Would this be considered a consultation because this physician is providing medical care not related to their mental health? Or would I code as regular E/M for the location type?

Any help is appreciated!

I was going to use CPT code 23472. However now I am wondering should I be using CPT code 23616?

inpatient substance abuse treatment

Hi, i bill for a hospital in NY that does inpatient substance abuse treatment ( detox and rehab), they also do medication maintenance once patient is discharged, and pain management for patients who have to use opiates to treat medical conditions. I have been searching for days on the correct way to bill these encounters. as in correct cpt codes, pos when admitted and during outpatient visits. For all inpatient we are using cpt codes 99221-9922 and 99231-99233 with pos 21 for all admitted detox and rehab patients. Then for the medication management we use 99201-99205 or 99211-99215 with pos 23? can anyone tell me where to find help at.
