Wiki Incomplete Spontaneous Abortion

Hi,

Can anyone please clear this doubt regarding o03.4 series.

Chief Complaint is vaginal bleeding.

The patient was diagnosed with Incomplete spontaneous abortion and has a past medical history of hypertension, diabetes, and obesity. Are we supposed to code the past medical history conditions as combination codes with O series or just code as I10, E11.9 so on…

The ultrasoud report states that there is no evidence of gestational sac or intrauterine pregnancy.

Wiki initial hospital visit with EPF history

Hello Guys ,

still a big confusion .can anyone help with this :

If all three key elements of the minimum initial hospital visit are not met, would you instead submit subsequent hospital codes ? (Ex: initial hospital visit case is leveling out to History – EPF, Exam -EPF and MDM – Moderate). Being the lowest level is detailed/detailed/low, I’m not sure how to proceed. Any insight appreciated, thank you!

Hi There. can anyone help me solve a scenario :
A patient is admitted to the hospital with the following diagnoses: On the day 1 : Cellulitis and Abscess of the left leg due to unspecified E.coli, After 2 days of admission he develops Septic shock, and Acute Renal Failure and encephalopathy due to the sepsis.

can be coded like the below:
1. PDx – L03.116, L02.416 (Local Infection)
2. B96.20 ( Reason for cellulitis)
3. A41.51 ( sepsis due to cellulitis due to E.coli )
4. R65.21
5. N17.9
6. G93.41

please help me with sequencing and concept of infectious agent coding

Wiki Tears – Traumatic or Degenrative

Hi Everyone, I need a clarification on DX to code for tears involving in joints.

How to code DX for tears when document provides mising info about Traumatic or degenerative?
When we go by index, it leads to Traumatic as first, so should we go with the traumatic codes? or we should also check the patient age with comorbids inculding degenrative diseases (OA, Bursitis, Chondramalacia etc.) to code for degenerative tears if not mentioned as trauma?

Please provide any approved guidelines if possible. Thank you…:o

Medicare Pt receives 3 vaccines in the same visit and also Annual welllness is performed in the same visit – Flu, Pneumococcal and Hepatitis A.

Billed codes are as follows

99214 – 25
90670
90674
90632
90471
G0008
G0009
G0439

Medicare denied the cpt G0008, G0009 as"This service/procedure requires that a qualifying
service/procedure be received and covered" hence need coding assistance.

Please help!!!

Hello, we billed 36902 by itself and MCD-Alabama has been denying it for " M49: Missing/incomplete/invalid value code(s) or amount(s).
N59: Please refer to your provider manual for additional program and provider information." ANyone help me please…Thank you.

Hi Friends,

Can anyone help me in clearing my doubts.

1. Can i code diagnosis which has mentioned the term "versus".
Example: abdominal pain versus small bowel obstruction.

2. If the patient has encountered for wellness examination for the first time and he is established patient for the physician.
Can we code 99213(established patient office visit) along with 99385(new patient wellness) ?
