Can someone tell me when using allograft (20930) if it has to be documented in the Op Report how the allograft material was prepared? My documentation states where the autograft was obtained and that it was mixed with allograft. That is the only reference to the allograft.

Just need to make sure I am meeting the documentation requirements for the allograft.:confused:
