We have a patient who is trying to get accepted into the military and because years ago he claims the ER told him he had asthma the military is requesting documentation stating the patient no longer has asthma which the patient claims. The patient does not remember where he was seen and there are no medical documentation showing a history of asthma. He was seen and evaluated including PFTs and cleared, no sign of any asthma. So what diagnosis should be used? No history of asthma in the family. Should asthma be used even if we only hear it from the patient who states he never had to use the inhaler either? Thanks.
