Chest vs Breast
there is no radiology forum so I thought I’d see if any can help with this. Patient was referred for an ultrasound. She has had a mastectomy and had a lump in that area. Would you charge a breast or chest ultrasound?

Encounter for pre-op chest x-ray
Patient is having a chest x-ray to r/o respratory disease before inguinal repair. history CAD, HTN, and s/p CABG.
Results: no respiratory disease but positive cardiomegaly.
So far I got:

Z86.79 Personal history of other diseases of the circulatory system
I51.7 Cardiomegaly
71010 Radiologic examination, chest; single view, frontaL
but then I started thinking, can I still say personal history of circulatory with finding of cardiomegaly? Am I over thinking this?

Procedure Details
The risks, benefits, indications, potential complications, and alternatives were explained to the patient and informed consent obtained.
The palpable lesion and surrounding skin was prepped with alcohol and betadine and draped in the usual sterile fashion. A scalpel was used to make a 2 cm incision over the mass. Staying close to the fibrous cuff, it was excised sharply. Hemostasis was achieved prior to closure. The wound was closed with 4-0 nylon and dressed with neosporin gauze and tape. The specimen was not sent for pathologic examination. The patient tolerated the procedure well.

Findings: Retained cuff
Condition: Stable
Complications: None
