Condyloma Excision Multiple

I am having trouble deciding the right way to code this. The below information is all I have to go on.

Procedure: Condylomata Excision x 6: Injected lidocaine before procedure and applied silver nitrate after.
Vaginal Biopsy: at introitus bx of lesion concerning for dysplasia

Assessment: Condyloma, Vaginal Mass

The provider has listed an E/m along with the procedures. My main concern is finding the correct code for the condylomata excision. There are no measurements for these, I’ve checked with the provider and she confirmed none taken.

Any guidance is appreciated.

Condyloma help????

This has me a little confused for the treatment of the condyloma.

Pre-op Diagnosis: CONDYLOMA PENILE/ GROIN

EXCISION CONDYLOMA PENILE/GROIN (N/A) – Wound Class: Clean Contaminated

Procedure: Patient is brought to the operating room and placed supine. After adequately induced general anesthesia the genitalia and surrounding area are prepped with Hibiclens and sterilely draped. Very large condylomata in the left groin region is noted. This is approximately 5 by 1.5 cm in size. It is excised using electrocautery, both cutting and cauterized current. There is a no other 4 x 2 cm lesion at the base of the right penis that is excised in a similar fashion. A 3rd large lesion on the left base of the penis is excised. The CO2 laser power of 5 w was then used to laser the base of all lesions so that the vaporized areas carried down to the dermis to prevent recurrence. There is a another 3 mm lesion on the left base and several smaller lesions in the suprapubic area that are vaporized with the CO2 laser as well. Minimal bleeding was encounter. Neosporin ointment is placed in all areas of resection. The patient tolerated procedure well was taken to the recovery room in good condition

infrared coagulation for Condyloma or lesions

My Dr decided to treat a well known patient with years of condyloma today with infrared Coagulation. The only Infrared Coag code is for Hemorrhoids but if you google there is many…. articles that states IRC is FDA approved for treating AIN, VIN, LSIL, HSIL, condyloma, pre mal lesions etc….. but I cant find anything that would be used for that besides the 46930 (which states destruction of internal hemorrhoids) Any suggestions?

Consent was obtained. The procedure and risks were explained in detail. Questions were encouraged and answered.

1% lidocaine with epinephrine, 1 to 2 mL was injected into the skin for numbing.

A single premalignant lesion on the RA was destroyed. In addition, 2 premalignant lesions on the Left lateral genital crease were destroyed.

Today, 3 benign lesion(s) located on the anus were destroyed with infrared coagulation.

Comments: The 3 lesions were injected with lidocaine 1% and Marcaine 0.5% prior to the use of infared cauterization at setting of 2.0 to each of the 3 anogential condyloma. Patient tolerated this procedure well..
