Different Self Pay Rates

Hi guys!

So it seems my boss is in the business of offering a different self pay rate to patient’s that have transferred over from his old practice. We are a physical therapy office, and our rate at our practice is about about 45 bucks more for each visit than at his old practice. He is giving the old practice rate to transferring patients (so a discount of 45 bucks each appt) – this has to be illegal, correct? What can I cite to him to get him to stop giving these patients discounts?

And just to be clear, he can treat them for free, correct? Just not give them a discount? Any help is appreciated!

Preventive or problem-focused visit, this is a different scenario

Not sure how to code this visit. 9 year old patient had a well child visit on 07/30/2018, but would not let the female provider perform the genitourinary exam due to history of sexual abuse. Returned on 08/08/2018 to have this exam performed by a male provider. Male provider also explained puberty and physical changes the patient would be experiencing in the next few years. Does this warrant a preventive visit or problem-focused? Can we code a preventive visit that soon? Can I code a problem focused e/m with Z00.3 and Z62.010. Any thoughts would be greatly appreciated.

22633/63047 same interspace same session same day 2 different specialities

Can anyone find the most up to date article from Medicare giving instruction on how to bill these codes If Neuro specialist Dr. A bills 63047 and Spine specialist Dr. B bills 22633. Should this be a co-surgery or is it appropriate for them to bill their own codes separately even though these are bundled codes per CCI edits? Hoping someone can help. I have researched every resource I have. thank you

Re: Codes O60.12-14 (different trimesters)

Hello, can someone explain to me descriptions in codes O62.12-14?

My basic understanding of trimesters are:

First Trimester, first to third months, 1st to 13th weeks
Second Trimester, fourth to sixth months, 14th to 26th weeks
Third Trimester, seventh to ninth months, 27th to 40th weeks

So if the patient goes into labor with delivery on her 35th week, I would use O62.14 Preterm labor third trimester with preterm delivery third trimester?

What I’m not understanding is the description repeating the trimester.

Example:
O62.12 Preterm labor second trimester with preterm delivery second trimester
O62.13 Preterm labor second trimester with preterm delivery third trimester
O62.14 Preterm labor third trimester with preterm delivery third trimester

Please help!
