Followup renal Ultrasound on 40 days old infant

Hi, I got a situation here and hope I can get a solution through this forum.
A 40 day old infant was ordered a Renal ultrasound as a followup on her abnormal prenatal US as they showed kidney dilation. We got a denial as we used O28.3 which should go on mothers records but not on an infant’s. Of course, we realized after the denial. The new born does not show any signs or symptoms to reorder this US for us to code and the intention is purely for re screening as per the order.
P.S- P09_Abnormal findings on neonatal screening was actually coded on the initial birth claim. What would be appropriate diagnosis code to code in this scenario. Thank you!

What code to use after 90972 for follow-up services with same doctor

I am seeking guidance on which code to use the day(s) after 90792 for a follow-up with the same doctor that did the 90792. We were once instructed to use 90791 for the follow-up and now that I’ve helped my supervisor to understand that this is not appropriate, I’m having a hard time figuring out exactly which code would be appropriate. I was thinking 9923*. The patients are usually in outpatient/ inpatient care for substance abuse at a facility and we are billing as a doctors group. Any guidance would be greatly appreciated.

Shared Visit: Doctor treats new problem & mid-level treats follow-up problem

CMS has stated explicitly that if a shared visit is not incident to, it must be billed under the mid-level. But one of my doctors is questioning why we can’t bill it under him if the patient has two problems: the mid-level provider only treats follow-up problem, and he (the doctor) evaluates and treats the new problem.

Technically, this is a shared visit, and it is not incident to. Yet it seems that it would be perfectly fair to bill the visit under the doctor, since the mid-level is only performing that portion which is incident to. Can we do this?
