88361 Looking for guidance

our providers bill 88361 x4 on gastro samples; the antibodies are MUC, P53, AMAR and CD. We are getting differing opinions. Some say do not use 88361 for any of the antibodies, some say to only use 88361 for the P53 and others say to use 88361 for P53 and CD.

When it is appealed to Medicare with MR they sometimes pay all 4 units, sometimes deny all saying the antibodies do not qualify for the code and sometimes they will pay one unit.

Any assistance or advice is appreciated.

Looking for Guidance for CGx billing for FCSO (Florida Medicare)

Hi,

I recently received denials for upper tier moldx codes because I billed two units per line. I had someone call a rep and the rep indicated they were denied because we billed more than one unit per line, and advised to only bill one unit per each CPT per line. Most of my experience says Medicare has a daily limit of two of these units. Is this something particular to this MAC? Looking for any guidance. Thank you.

Looking for E/M guidance in Ortho Exam

Would you say the following is a Detailed or Comprehensive exam? I’m currently in discussion with one of our physicians who is arguing that he should get more credit under Lumbar Spine and Motor Strength sections below as he examined the lower extremities. My opinion is that since he’s examining the spine, the pelvis is included in the spinal body area. What is correct?

Ht 5’7", Wt 192lbs, BP 98/66 Sitting (+1)
Constitutional: General Appearance: healthy-appearing and NAD. (+1)

Psychiatric: Orientation: oriented to time, place, and person. Mood and Affect: active and alert.(+2)

Gait and Station: Appearance: limp and antalgic gait and ambulating with no assistive devices.(+1)

Cardiovascular System: Arterial Pulses Right: posterior tibialis normal and dorsalis pedis normal. Arterial Pulses Left: posterior tibialis normal and dorsalis pedis normal. (+1)

Skin: Lumbosacral Spine: normal skin. (+1)

Lumbar Spine: Inspection: no induration, ecchymosis, or swelling and normal alignment. Bony Palpation of the Lumbar Spine: no tenderness of the spinous process or the sacrum. Bony Palpation of the Right Hip: no tenderness of the iliac crest, the SI joint, or the greater trochanter. Bony Palpation of the Left Hip: tenderness of the SI joint and the greater trochanter and no tenderness of the iliac crest. Soft Tissue Palpation on the Right: tenderness of the iliolumbar region. Soft Tissue Palpation on the Left: tenderness of the paraspinal region at L 5 and the iliolumbar region. Active Range of Motion: some limitation of lumbar motion.(+2)

Question Fluroscopy Guidance with CPT 64650 or 64450

Our Providers are asking if they can bill 64650 with 77002 – according to AAPC coder 77002 is an add on code and cannot be billed with 64650. Also they are asking if they can bill 64450 with 77003 again 77003 according to AAPC 77003 is an add on code and cannot be billed with 64450. Are there any Fluroscopic Guidance codes that can be billed with these procedures?
