Does anyone know if in box 31 it is possible to have "Signature on File" or does it have to be the Provider’s Signature and Credentials?

HCFA 1500 Question
I am hoping for some clarification regarding 1500. Do 24J and 31 have to match? Can multiple providers be listed on a single 1500 form? Any documentation regarding this would be much appreciated!
Record Release and HCFA 1500 Claim Forms
When a patient requests to have a copy of their UB-04 or HCFA 1500 claim form for AFLAC, do we need to get a Medical Records Release filled out by the patient? I am getting mixed reviews saying that it is part of the medical record and others saying that it is a financial document and a release is not needed. I just want to be in compliance but not create extra work. Any input or website links would be great. Thank you.
