I have a claim that was not paid by Medicare for a patients routinely scheduled remote 90 day device check (93297 and 93299). The patient was inpatient at one of our facilities at the same time (for another reason), by our Cardiologist billed a visit for that date too. A/R is stating that we cannot bill for the remote check since the patient was in the hospital. I don’t recall coming across this issue before…….any input/clarification/resources from my fellow Cardio coders would be appreciated.

Reimbursement for E/M for Illness at Wellness exam
Anyone else having problems with FULL reimbursement for the OV (99212-99214) when billing it with a wellness exam (99391-99395)? Reimbursement was always @100% as long as it was coded with modifier 25, now we are only being reimbursed 50%.

E/M code for 1 chronic illness that is stable
If a patient has a DM or hypertension and they come in for their follow up visit and the illness is stable, does this qualify for a 99212 or a 99213 (if, in this scenario, the exam level meets either a 99212 or a 99213)?
