Developed by the Lucile Packard Children’s Hospital at Stanford in Palo Alto, California, the matrix is combined with the hospital’s electronic medical records system to allow quick assessment of patients and the types of transportation needed to evacuate them to safety. The matrix is also available in PDF form online.

Bone Graft application (DBM bone matrix)
Hi everyone,
Provider performed an open reduction with application of external fixation for left calcaneal fracture, also an application of DBM bone matrix bone graft. To report these procedures:
(1) 28415 LT – open treatment of calcaneal fracture
(2) 20692 LT – external fixation
However, not sure which code to use to report the bone graft, it was not an autograft. If anyone could advise what code should I use to report the bone graft?
Thank you very much!

SAFER in practice; Thoughts on Joint Commission’s new scoring matrix
There’s been much ado about The Joint Commission’s new scoring system, the Survey Analysis for Evaluating Risk (SAFER) matrix. But, unless you happened to be one of the facilities surveyed in the last four months, it means that you still haven’t seen SAFER in practice. So what are people saying about the new system? Is it better or worse than the one that came before?
Victoria Fennel, PhD, RN-BC, CPHQ, director of accreditation and clinical compliance at Compass Clinical Consulting, says Compass has heard from clients who’ve experienced the matrix and most of the feedback has been very positive.
