A patient came in and had three seperate lesions removed by shaving full thickness. All on the back two the same size and one larger. The provider coded as 11300,11300-59 and 11301-59. I feel this is correct. There is some confusion in our office with the "new" biopsy codes that came out this year 11102-11107. Someone said since they are on the back they should all be added together in one code. I said that was for laceration repairs. Can I get some help with this please as they were all full thickness lesions removed all benign. Thank you

Callus shave
Can codes 11055-11057 be used for billing a shaving of a callus? The procedure note states callus shaved with #10 scalpel. I am thinking that a shave of a callus is included in the E&M code and 11055-11057 should NOT be used since the procedure was a shaving and not paring or cutting. Any thoughts or suggestions?

Shave removal with destruction of benign lesions
The provider documents that the lesion is removed via shave technique, but also is destructed and the lesion is confirmed benign. We are trying to determine if we should bill the shave removal code (113xx) or benign destruction (17110). According to NCCI we cannot bill both the shave removal and destruction codes as they are bundled into the destruction. It seems like it would make more sense to bill for the shave removal than just the destruction.
