Why 99213? Only one component matches. (Chapter 17 Table G., Official guide)

This is from 2015 Official guide, but would be the same for 2017 guide.

Chapter 17 E/M, Table g,

History: Detailed
Exam: Expanded problem focused
MDM: Straightforward

It shows two of three components should match to make level.

But, only one, "Exam: Expanded problem focused" matches 99213.

What is the other component matching 99213?

Any idea?

MDM Table B – Data Points

HI Everyone

I have a doubt in MDM B table Related to the point, Independent visualization of labs or image – 2 points.

In chart

MD independently viewed and interpreted the Chest X-ray and also independently viewed and interpreted the Ultrasound lower extremity. Kindly guide me shall we consider it as 4 points or only 2 point because both are from the radiology section.

Urgent !!!!!!!!!!!!!!!!!!!!!!!! Please help!!!!!!!!!!! MDM TABLE A

Hi guys,

I need an advice as soon as,
A patient came to Dr. A (MD) for Cold sore, and Prescribed with Rx , Later he came for Refill to the same practice and examined by PA for cold sore and make a refill. I just want to how we can count the point for MDM-TABLE A, whether it is New problem or established problem….

As per the CMS guidelines, this problem should be considered as an established to Dr . A, but this is a New problem for PA, since now only he met this Patient ….

How we can code this… I need a help on this….

EGD: Endoscopy with/ without risk factors on Table of Risk

Hello,

I was wondering if an EGD, which does have risk factors (perforation, bleeding, infection) but the potential of these risk factors is slim to none, would that still count as "endoscopy with risk factors" on the Table of Risk (level 5 MDM)?
