Billing 99213, 11721 & 11055 Humana

I am having a hard time getting the any office visit code (99212, 99213, 99214) 11721 & 11055 all paid by Humana. I have always had trouble with this insurance bundling these services and only paying for the lower dollar amount item (this example 11055 was the only one paid). This is how I billed the original claim to Humana:

99213 25 Modifier M20.5X2, M21.6X9, E11.40, M20.41
11721 59, Q9 Modifiers E11.40, I73.9, M79.675, M79.674
11055 LT, 59 Modifiers L85.1, M79.675, E11.40

What am I doing wrong? 2018 I only had a few problems with Humana bundling the codes, this year its all up in the air. Please help me so I can get my provider paid. Thank you in advance! :)

99213 versus 99214

I always feel stressed when we have a new provider come on board with our facility and we have differences on E/Ms. In this case the provider coded this visit as 99214. I down coded it to a 99213 based on the following (follow up on one problem worsening or not responding to treatment, HPI, and time spent with patient). I do thing the MDM is Moderate but I am not sure that trumps everything else? I would appreciate your opinions!

75 year old female who is 1.5 weeks post-op from aortic valve replacement (TAVR), with history of peripheral neuropathy, multiple spinal surgeries, Addison’s disease, atrial fibrillation s/p AICD/pacemaker on warfarin, CAD here for left leg pain and increased swelling.

-no lightheadedness/dizziness
– has had palpitations the last few days; had them most recently this am.
-feels like she is getting adequate air; no shortness of breath
-no chest pain
-left leg swelling from knee down worse than usual

– no history of DVT or PE that she can recall.

Assessment and Plan: 75 year old female who is 1.5 weeks post-op from aortic valve replacement (TAVR), with history of multiple spinal surgeries, Addison’s disease, atrial fibrillation s/p AICD/pacemaker on warfarin, CAD here for left leg pain and increased swelling. Given recent surgery and subtherapeutic INR, there is concern for DVT/PE. However, patient’s leg swelling is relatively unimpressive with no erythema, warmth, collateral veins or significant enlargement compared to other side. I reviewed discharge summary and records from her recent hospitalization. Per her thoracic surgeon, unclear benefit of warfarin in this situation, and duration of therapy also unclear. No shortness of breath now and VS are within normal limits, making pulmonary embolism less likely, but she has had palpitations last 2 days. Other etiologies of palpitations could be cardiac arrhythmia such as rapid atrial fibrillation, dehydration, anxiety. None of these are apparent today.
– LLE duplex now.
– shared decision making around CTPA – patient declines at this time and I think this is reasonable – see above.
– will rx lovenox if US shows DVT.

I can’t remember, I am thinkng I can not bill 99213 after we sent to ER and in/pt ad

I am having a memory blockage. We had a pt come in(work in) and my doctor decided to send to ER (at our smaller county hospital), patient had more than they could handle and sent pt to bigger hospital and was then admitted for an IN Pt stay now being seen by internist and oncology. Am I able to bill for the office visit 99213. Our office note and the local ER doctor both say PCP sent pt to hospital.

Why 99213? Only one component matches. (Chapter 17 Table G., Official guide)

This is from 2015 Official guide, but would be the same for 2017 guide.

It shows two of three components should match to make level.

But, only one, "Exam: Expanded problem focused" matches 99213.

What is the other component matching 99213?

E/M Dermatology 99212, 99213

We have had some difference of opinon on how to bill out the E&M lately with new drs on staff vs drs that have been here for years. Generally patient comes in here for a Full Skin exam which may or may not result in some biopsies but mostly the only other things notes are SK’s and maybe Acne or something. What level would most of you see?

Also in the case of patient presenting with Alopecia dr looks it over perscribes medcine or treatment plan patient goes home?

All notes are very basic and straightforward
