New chest x-ray CPT and Abdomen

Good Morning Everyone,
I wasn’t sure if anyone was having that same problem that I am with new radiology CPT codes for 2018. I have not received any payments from all insurance companies on the new CPT codes (Chest) 71045, 71046, 71047, 71048 and (abdomen) 74018, 74019 and 74021. They have all denied. Is there any news as to why these are all denying and what have you done about it.

Coding X-ray

My provider’s x-ray machine is broke therefore we are sending patients to another facility to have x-rays performed and they are bringing the x-rays back to have them read by my provider. If the facility is charging my provider instead of billing the insurance directly, can we bill the insurance for the x-ray or are we only allowed to bill for the reading?

thumb x-ray coding

I am looking to code a thumb x-ray but I am finding conflicting information. Some resources state that the thumb is considered a finger and should be coded as such. Others state the finger x-ray code is for digits 2-5, excluding the thumb which is part of the hand and should be coded with the hand x-ray CPT.

Can anyone clarify this please?

What CPT code would you use for this?

Study: Standing RIGHT lower extremity radiographs
Technique: Standing AP radiographs of the RIGHT lower extremity obtained at 72 inches. Radiographic markers are placed around the knee
Additional clinical data: 71-year-old with RIGHT knee pain and primary osteoarthritis for presurgical planning for RIGHT knee replacement. Visionaire protocol

Findings:
There is no evidence of fracture or destructive lesion.
Prominent osteoarthritis changes are noted within RIGHT knee joint, especially along the medial compartment where there is prominent joint space narrowing, subchondral sclerosis, and osteophytes. There are extensive intra-articular loose bodies and/or foci of heterotopic ossification noted along the distal RIGHT femur. Mild to moderate osteoarthritis change

Impression:
No acute osseous pathology by radiography. Severe osteoarthritis of the RIGHT knee, most prominent within the medial compartment. Heterotopic ossification versus sizable intra-articular loose bodies along the distal RIGHT femurs are noted within the RIGHT hip joint.

Shots taken Vs Views shown Xray

Hi,

I was wondering if it would be compliant to do the following? We felt that taken less actual shots would provide the patients with less radiation when the extra shot is not necessary. Unfortunately now there is question as to whether or not we should be billing the way that we are.

A patient presents and a two view foot and a two view heel (calcaneus) are ordered.

The xray tech takes two shots of the foot AP and Lateral and one shot of the Heel axial as the lateral heel is already visible on the shot of the lateral foot so another shot is not required to see the lateral heel…….therefore saving the patient further radiation (though I know that it is slight).

We still have all 4 views that were ordered and felt comfortable billing with the 2 view terminology for each code (73620 and 73650) as all views are present.

Now we are being told that we should be billing for how many shots are taken, not how many views we have.

Does anyone have any documentation to support either side of this argument? I have not been able to find any.
