The Coder’s Desk Reference states, "Report 28305 when a bone graft is necessary. For this procedure, the physician debrides the intended graft recipient site of the tarsal and a bone graft from the iliac crest or other site is shaped and placed between the prepared surfaces."
So am I correct in thinking that based on the Coder’s Desk Reference that the autograft can’t be locally harvested and has to be harvested from a different area?

I work in an ASC and I have a coding question on Pterygium. Below is the procedure performed.
OPERATION: The patient was brought to the operating room in good condition. After undergoing peribulbar block by the anesthesist the right eye was prepped and draped in the usual sterile fashion, a lid speculum was placed. Inspection of the cornea demonstrated nasal scarring, which was released using 0.12 mm forceps and Vannas scissors. Next, the pterygium was completely excised along with excess Tenon’s tissue and release symblepharon formation nasally. The resulting defect was then cauterized to achieve good hemostasis.
Finally, a thin area of conjunctival tissue was harvested superiorly and glued over bare sclera. Routine postoperative drops and ointment were placed in the eye. The eye patch and covered with fox shield.
what would be the codes for the above procedure and modifiers, if any? Please help.
