Colon Cancer Screening Guidelines

How specific are the guidelines for screening? I know it’s every 10 years, but is it exact date to exact date (1.1.08 – 1.1.18) or just by calendar year? I had a pt come in for a screening on 2/6/18 and previous was 12/29/08 which technically is less than 10 years. Any thoughts/clarification is appreciated.

Capsule not reaching colon prior to completion of study

We had an instance were a patient had a capsule endoscopy done, the report states normal small bowel was visualized, but the capsule did not reach the colon prior to completion of study.

Billing wise- would this be reported as an incomplete? 91110-52

Screening for colon cancer with CTcare

We were just told that Connecticare is no longer paying for screening for colon cancer with an office visit. They told us to bill the visit before the colonoscopy with the dx of Z01.818, Encounter for other preprocedural exam as the diagnosis. Anyone else experience this? Are you still getting paid with Z12.11?

Two Polyps in Transverse Colon

Horizon BCBS has performed an audit on our procedures, and they stated that when two polyps are found in the same area(lets say transverse) and if we perform a snare on one polyp and cold forceps on the separate polyp, we can only bill the one with the highest RVU’s. I was always under the impression that as long as there are two separate polyps you can bill for both even if they are in the same area. Has anyone else come across this issue? I am now wondering if this is something that is across the board with all the insurance companies. I have been researching but can not find anything.

Cecostomy with decompression of the sigmoid colon

Hello,

The physician performed an open sigmoid resection with end colostomy. I have code 44143 for this part of the procedure. He then did a cecostomy (separate incision into the cecum with closure afterwards) with decompression of the sigmoid colon (milked out the contents).

Any ideas on how to code the cecostomy with decompression? The only codes I can find are with the colonoscopy and sigmoidoscopy.

Any suggestions would be greatly appreciated.
